As computer technology has touched
so many aspects of our everyday living; it now too has entered the
realm of healthcare. When only a short time ago telephone was the
quickest way to contact patients, doctors around the world are now
beginning to practice medicine through internet access.
I believe the internet can become a valuable tool
between patient and doctor if used correctly. That is why I have
set up guidelines for patients and myself to follow as we learn
and fine-tune this innovative way to work together.
Piatek institute’s internet/e-mail patient
guidelines:
- Must be an established patient (with a completed
medical file) of the Piatek Institute/Dr. Piatek.
- Patients must have no complicated health risk
factors as determined by Dr. Piatek.
- Patients must be accurate and honest when
reporting all medical data required.
- The proper medical documents must be signed
before treatment is to begin.
If you have any questions, comments, or concerns

Note: Please do not use e-mail for critical
medical decision-making, or any type of emergencies.
STEP
1
On your e-mail appointment day please
E-Mail your:
1.) Weight
2.) Blood pressure (if needed)
3.) Amount and type of exercise
4.) Appetite control
5.) Any questions, comments, or complications
you may be having
*lf your e-mail date does not suite you, e-mail
us as soon as possible and let us know the date that is best for
you.
*Dr. Piatek will respond within 24-48 hours with
his advice, recommendations and billing amount.
STEP
2
You will then need to send a second
E-mail to notify Dr. Piatek of your approval, or with any other
questions you may have before your medication/products will be sent
to you. *A simple OK with your name (once you have no further questions).
An e-mail back will give us the go ahead on your order to be sent.
All medication will be sent U.S. Priority Mail and will require
a signature for receiving (see address contact form).
If you have any questions please feel free to
call the office at any time at 317-243-3000. For emergency use
only you may contact Dr. Piatek’s pager at 317-906-3669!
Note: Please do not use e-mail for critical decision-making or

Note: If Dr. Piatek feels your appointment
is requiring lengthy consultation or explanation, he reserves the
right to speak to you by telephone or request an in person appointment.
